Application Closed

The application for the Chicago Police and Firefighter Training Academy (CPFTA) - Class of 2014 was closed to new applications on April 23, 2012.

If you have already completed the online portion of the application, all remaining application materials must be received in our office no later than 5:00pm on Monday, April 30, 2012.

Application Steps

1. Complete the Online Application
This part of the application process is now closed. If you have already completed the online portion of the application, all remaining application materials must be received in our office no later than 5:00pm on Monday, April 30, 2012.

2. Complete Your Consent Form
After submitting the application form below, you will be presented with an option to download your Consent Form. Please print the consent form (one page) and complete it (signatures and dates) in its entirety. If you need to get another copy of your Consent Form please click here.

3. Send In Your Consent Form and Your Documents
The Consent Form, along with the following documents:

  • Two Letters of Recommendation (one from a school official, one from a community leader)
  • Current Physical (must be from within the last 12 months)
  • Copy of student’s Driver’s License or State Identification Card
  • Copy of High School Transcript for G.P.A. and Attendance verification

must be received in our office no later than 5:00pm on Monday, April 30, 2012. An application is INCOMPLETE until the signed Consent Form and other documents are received.

4. Chicago Police Department Processing
Approximately 2-3 weeks after the Consent Form is received, the Chicago Police Department will mail you a packet which includes a "Personal History Questionnaire," a background investigation form, and an "Authorization for Release of Personal Information" form. These forms must be signed, completed and brought with you to processing day. Your processing date along with additional information will be in the packet.
