The Individualized Learning Plan (ILP) is a formalized plan and process that helps students develop concrete plans for success in college and careers. It involves students setting goals based on personal, academic and career interests, and meeting with counselors or postsecondary staff to review.

To promote preparation for and success in college and career for all students, CPS has designated Oct. 7-15 as Chicago Champions College and Career Week. CEO Jean-Claude Brizard has said: “A high school diploma is just the start – students need education and training beyond high school to have access to America’s middle class.”
On January 23 CPS will deploy the new online SIM Academic Planner, which will enable High School counselors to view course-specific contributions to satisfying graduation requirements, forecast graduation based on current course load, and identify non-contributing credits.. These system enhancements will provide High School counselors with unprecedented capability to track and guide student progress towards graduation. To successfully leverage the new Academic Planner and understand the associated course conversion impact on High School Transcripts, please familiarize yourself with the information here.
Former Student Records
The Office of Former Student Records maintains student records for the majority of Chicago Public Elementary and High Schools. With written authorization the department can provide a copy of a high school transcript, verify a graduation date from high school, copy of an elementary registration card or medical records.

Our School Counselor Practicum/Internship Program has been developed to assist CPS elementary and high school counselors in partnering with an intern in order to positively impact student achievement while exposing future counseling professionals to evidence-based academic, personal/social, and college and career programming within CPS.
To assist in developing students who are college and career ready, each high school will be implementing a process for the collection, use, and storing of Social Security Numbers (SSNs). Please refer to the Chicago Public Schools Social Security Number Protection Policy, Section: 102.9, Board Report: 11-0525-PO3 for additional information.
The Chicago Public Schools - Office of Pathways to College and Careers has partnered with The Illinois Student Assistance Commission (ISAC) and Parchment to provide the College Illinois Transcript Exchange, a system powered by Docufide by Parchment, which will enable CPS high school students to request their official transcript online for electronic delivery to thousands of colleges and other destinations.
